You can stress any of the three words to give slightly different meanings. The most common way to say it would be "How ARE you?" or to use no stress at all. But you could also stress the word "how" to get the meaning of: Not where are you, but HOW are you? Or you could stress the word "you" to get the meaning of: John, how are you? And Lisa, how are YOU?
