"You're welcome" is used to answer "Thanks" or "Thank you." In another context, it means "It is okay," (Example: You're welcome to stay / It is okay for you to stay)
I don't think there is a phrase as "Here you are" but I know "Here you go" which you say to someone when you're handing over something (like a borrowed pencil)
People usually say "No problem" aside from "You're welcome."
