Yes, all those questions are correct.

sorry, sorry, sorry... spelling mistake.... meant to write "Where did you first see him?"... my bad (terrible expression :).

I can't explain why, but "Where did you fist see him?" sounds better. What I don't know is if one or the other is more grammatically correct.
