Does the phrase 'hace fresco' mean 'cool' as in a weather sense or is it more similar to the phrase 'qué guay?'

"Fresco" means "cool" as in weather sense. It can never used as a synonym for "guay".

En España siempre decimos "hace fresco" para indicar una temperatura fría, pero sin llegar a ser desagradable.
En países de América del Sur la expresión “hace fresco” les suena un poco mal, y prefieren decir “está fresco”. Pero en España es al contrario, decimos “hoy hace fresco”, y no “hoy está fresco”

"Fresco" means cool, but "Hace fresco" doesn't have any sense. The right way to say it would be "Está fresco" e.g. "The day is cool" means "El día está fresco", "He is a cool guy" means "Él es un hombre fresco".
Remeber the verb to be can be used for two different verbs in Spanish, "Ser" or "Estar"
