Well, I’ll add some modifications, just for additional information:

-my first name is Amal, and my family name is Adhalum = mon prénom est Amal et mon nom est Haulm

-I'm from. Palestine : je suis de la Palestine.

-I’m 17 years old: j'ai 17 ans.

-I'm in the eleventh grade: Je Suis en onzième année. (Be aware the final ‘s’ of suis is silent)

Maybe if you made up a whole assay, it sounds better :-)
