No, it shouldn't be capitalised. You capitalise if it's part of a person's name or in 'Dear Sir'.  Otherwise, it's lower case.

And yes, you need the comma.

Sorry but I disagree with our friends here. In your sentence, sir is used as a name so you would capitalise it. It compares with, 'What do you think you're doing, Mum?'
And yes, the comma is essential. Note the difference between, 'Ive already eaten, Mum' and 'Ive already eaten Mum.'
