1. VA stands for Veterans Administration. A VA hospital, then, is one which accepts people who serve or served in our military. 2. I think that the use of hole here is metaphorical. I would guess that since he is in the hospital, his plans for the rest of his life will need to change, and he describes it as a "big hole" in the middle of his life. 3. Blown through means it entered, possibly forcefully. 4. Middle is simply not at the end nor the beginning. I hope this helps!

VA = Veteran's Administration. A VA hospital is a hospital for wounded soldiers, away from the battle scene. The hole is actual and metaphoric. A bullet has put a hole in his spine, crippling him. It has figuratively blown a hole in his life, because now he cannot walk. "Blown through" in this case means to pass through destructively. "The bullet blew through his leg." The "middle" in this case, is the main portion of the soldier's life. All other things in his life were centered on that fact that he could walk. Now that he cannot walk, a major portion (the middle) of his life has changed.
