When do you use und Sie and und Ihnen?
When you have this conversation
-Hallo, wie geht es Ihnen?
-Gut, und Ihnen?
I know if I say "Gut, und Dir?" it's more informal, but does it mean the same
[Gut, und Ihnen? and Gut, und Sie?] in this example?

It's a matter of declension. The formal "Sie" is just the third person plural pronoun, capitalized.
Nom: Sie
Gen: Ihrer
Dat: Ihnen
Acc: Sie
Possessive pronoun: Ihr/Ihre/Ihr usw., see the usual table
