Do Japanese people get confused when reading kanji? Or do they get it right away?

It is the same thing that people hardly remember those vocabulary which is not used often. Basically, adults do not get confused when reading Kanji unless it is commonly-used. I am sorry for my English, I am now in progress to learning.
