I recommend you check the guidance on the university website, and also book a lesson with a teacher or tutor to improve your effort. Even if your English is good, I recommend you still get feedback to improve your style and so sell yourself well.

I have no idea. I just did a google search, and it appears to be something that American Universities require. There seems to be plenty of references online.
