you have a childish smile..
I have a question about this word "childish".
Does it has a negative meaning like stupid?
Is it weird to use 'childish' as the meaning of 'innocent' or 'pure'???
What would you feel if you listen to " you have a childish smile"?

I'd say "sweet" in this context. A sweet smile.

My opinion is that childish has a negative connotation. If I want to express the innocence and purity of childhood, I would use the word childlike.
