How can I write this sentence into a formal question?
vous vous étes levés tôt.

You have to reverse the subject (vous) and the verb (se lever) :
==> Vous êtes-vous levés tôt ?
It is quite same as English ==> Have you waken up early ?
Past participle is logically located after the subject.
