Comment as-tu fait ? est la meilleure forme écrite. Comment tu as fait ? est davantage utilisée oralement, mais est moins correcte.

The proper way would be "Comment tu as fait?". "Comment t'as fait?" is not correct but use often use in spoken language. It's informal.
