Je pense que le mot correct et apparent, non ?

I think that 'apparant' is correct. It's a present participle used as an adjective - the phrase means 'It's the first apparent/visible sign of dental decay.' I'm not a native speaker, but that's my understanding of the sentence.

C'est le premier signe apparEnt. "Apparent(e)" est un adjectif qui signifie clairement visible.
