It can mean one of two things:
1) Arrange a meeting (date) with the opposite sex
2) I will help you acquire what you are looking for

"Hook you up" is slang for "get you what you need / want "

It can also mean you can get something somebody wants for a cheap price.

It means: I'll bring both of you together (arrange a date or something).
