Your clock/watch is five minutes slow/fast?
Let's say in your time zone, it's 5 pm now but your clock or watch says it's 4:55 pm. Is it natural to say your clock/watch is five minutes slow?
And if your clock/watch says 5:05 pm, is it natural to say, your clock/watch is five minutes fast?

Yes, it's natural to say:
"My clock/watch is five minutes slow" if your clock/watch says 4:55 pm when it's actually 5 pm.
"My clock/watch is five minutes fast" if your clock/watch says 5:05 pm when it's actually 5 pm.
