You can pronounce V and TH correctly but you need to do so consistently. I would make that your primary focus. In particular, try to keep your V sounds different to your W sounds. "Visit" should not sound like "wizard". You may also want to double-check the vowel sounds in "chat" and the first syllable of "patient".

I'd say mostly work on vowel sounds. I'm not sure if you are trying for more of a US or UK sound, but listen to whatever you are after and imitate it. Also, maybe it's because you're reading a poem, but the rise and fall of intonation makes it a little harder for me to understand.

It's a nice, non-specific sounding accent, nothing hugely jumps out at me. Maybe just the vowel sounds in general. Google translate has very accurate text-to-voice, you can always use that and try shadowing/mimicking the pronunciation or checking the pronunciation of words.
