1) a no-account with a perpetual three-day growth on my chin.
A no-account means a worthless person (US English). Perpetual three-day growth on my chin means that a person has given up on life and this is reflected in the fact that he hasn't bothered to shave for 3 days.
2) barely hung on to my life
This sentence means that he nearly died. In this case from Cancer.

In addition to the person before me “three day growth on my chin” can also be referring to growing a beard or being unshaven. He’s essentially saying he would have nothing going for him.
