I have read its meaning on dictionary's website,but I'm still understand.
For example
"My last therapist concluded that I have deeply rooted fears of abandonment, that stem back to a really difficult relationship with my father."
What does stem back in this sentence mean?

It means “come from”. So the person’s abandonment issues come from the difficult relationship with their father.
Another way of putting this would be “because of my difficult relationship with my father, I have abandonment issues.”
Does that make more sense?

In this context, " that stem back to" means "that exist as a result of" or "which are due to." In other words, the difficult relationship is the root cause of the fears.
