If you do something by hand, it means that you do it manually, i.e. without machines. E.g. if you add something up by hand, you are not using a calculator.
If you make something with your hands, it means you use your hands to make it.
So these two phrases are slightly different. But e.g. making a cake by hand ends up meaning pretty much the same as making a cake with your hands.
