It means "truth" only in the sense of "revealing true feelings." Normally, we have a sense of caution, or social awareness, that makes us think about consequences, and inhibits us from expressing our feelings if we know that they are going to be rude, or provoke anger, or hurt somebody else's feelings. Alcohol tends to remove those inhibitions; people say what they feel without thinking about the consequences.
