I feel below sentence lack of "is" should be " is perplexed", right?
# His wife perplexed by his breathless appearance.

You're right that it's definitely not a complete sentence without a verb. One way you could make it a sentence is by adding "is", "was" or "will be" before "perplexed."
You could also place the verb after the whole structure:
His wife, perplexed by his breathless appearance, asked him what he had been doing.
