2.The teacher is reading the paragraph loudly, the students are following.
3.The vacation starts (started after) the exam (had) finished.
4.The boy was knocked over, blood (was) streaming down his head.
5.We fell asleep,(with) all the lights on.

While they are sort of ok, they would be much more normal sentences with the words in () and no commas.
