In long numbers, only the final one is ordinal.
53rd - fifty third
101st - one hundred and first
2,355 - two thousand three hundred and fifty-fifth.
1,000,001 - one million and first

My own American take is you may as well include the "and." You don't need it, but it looks slightly better. If you're reading it out loud, you wouldn't say "and." That would sound informal in that case.
