The second one is correct: "I would be glad if you would accompany me."

Both are ok. It’s not wrong but I don’t like saying “would” twice. I like “it would be nice were you to accompany me”.

To me the second one sounds more natural, but if you used the first I would say accompanied to make it sound more natural.
