Hola Juan Manuel. Cuando contamos el tiempo que llevamos haciendo algo (always - desde siempre), utilizamos el tiempo perfecto: have + participio pasado. Aunque en español se dice miedo (nombre), en inglés utilizamos el adjetivo - afraid.
