Hi, Could you help me with this,please?
Considering today is Tuesday, If I say "On" Sunday morning I had a doctor's appointment , or "last Sunday morning" I had a doctor's appointment .
Do they refer to two totally days or not?

Both are perfectly correct. By using ON you are presuming the listener will know it was the previous Sunday.
If you want to avoid the possibility of any confusion then use the more precise word LAST.
