“I’m prone to making mistakes” would definitely be the correct choice. With “prone” you need a verb plus -ing. Another example would be “My grandmother is prone to falling in the shower.”

It’s “I’m prone to make mistakes”, but I’d just say “I tend to make mistakes”.
