It means: How are you doing?

It is simply asking you how you're doing. If you happen to be new to a certain place, then this person may be asking you how you're coping with your new environment.

I agree with Eliot and KG. Just another thought. In America we can use this term everyday to just mean "How are you doing?" However, it is often used when we know or believe the person is having a difficult or anxious time, so, at this point in time, we expect them to have negative feelings. You might ask this of someone who is sick, someone who is new to a job or environnment and is not yet used to it, someone whose parent died, someone who was just arrested, etc.
