"I've been well" or "Not so well"
"Fine, thanks"
"Good, and you?"
"Not too bad"
"Ehh, I've been better"
--> It all depends on how you feel, how polite the situation is, and your personality. Generally people don't want to know much about what has been really happening with you, or if you are having a bad time, unless it's a relative or good friend asking.
