It's the feeling you get in your stomach when you are nervous. It feels like something (or things) is moving inside your stomach. It is a physical reaction to fear, nervousness and stress.
It is not always bad. People who are in love initially say they have butterflies in their stomach when the object of their affection is near. It doesn't last - thank goodness. Think of what it would be like to feel that all through a marriage.
