Silas is one of the characters in The Da Vinci Code. He wears a metal device around his thigh to cause himself pain - part of his religious outlook. This "thing with the teeth" is called a cilice (look up the images).
You'll see a cilice has prongs or hooks. The speaker says "teeth" because they bite into you.
