People generally say that when a romantic couple is having problems. (Sometimes for other things, but mainly relationships.)
For example:
Person 1: "My wife usually kisses me when I go to work, but today she didn't even say bye!"
Person 2: "Uh-oh, sounds like trouble in paradise."
