How are you?
is a form of "contracted" of "how things work" or "how's life" and, in theory, can be translated literally as "how's everything?"
How are you?
is the most common form and is the equivalent of "how are you?".
The meaning is slightly different, but are used in common speech in the same sense, that is to ask someone "how are you" in general.

Come va?
è una forma "contratta" di "come vanno le cose" o "come va la vita", e, in teoria, si può tradurre letteralmente con "how's everything?"
Come stai?
è la forma più comune ed è l'equivalente di "how are you?".
Il significato è lievemente diverso ma vengono usati nel parlato comune nella stessa accezione, ossia per chiedere ad una persona "come sta" in generale :)
ps, va è sempre minuscolo in questa forma.
