He is ____ a jacket.
A. carrying
B. holding
C. having
D. wearing
Note: Is this a poorly constructed question or is it just me?
It seems there are multiple correct choices except probably for C. (Because I don’t know what it means to say someone is having a jacket.) What do you think?

You're right, it is a poorly-constructed question. The author probably expects the answer to be D, but A and B are technically correct.

Just to add. You are correct. C is a poor choice.
