Please can you explain how 真的假的 works
I saw this sentence today '真的假的?' The translation given was 'Is that true?'
How does the grammar work? When I first read it I understood it as 'Is that really not true?
Meaning I read it to be：真的Really （是）假的not true（吗） ?'
Should I read this more like a short version of maybe 是真的吗？是假的吗？

It's a very colloquial way to ask "really, is that true?" (with a slight sense of disbelief, sort of like "no way!" or "you're kidding!" in English). For example, if someone claimed to have scored a perfect 100 on a very hard test (that almost no one gets perfectly), you might say, somewhat doubtingly, "真的假的？”
Your intuition is right about what a longer form of the expression might be. It could also be a shortened form of "是真的還是假的？？？”

台灣通行 國語(華語) 之外 閩南話 (狹義台語) 及 客家話 是二大主要方言
無論在 台灣閩南語 或是 台灣客家話 裏頭 都有 "真抑假" 這個詞
抑 : 連接詞 或、還是之意。表示選擇關係。
例：
閩南話: 是你欲出國抑是伊欲出國？Sī lí beh tshut-kok ia̍h-sī i beh tshut-kok?
(是你要出國還是他要出國？)
"真抑假" 這個詞 滲入國語後 就被譯成 簡短形式的 "真的?假的?"
也就是 "是真的嗎? 還是假的?" 之意

in fact, this whole phrase is : （这是）真的（还是）假的？---is this true or false?
so we use simply 真的假的？ to express "really?" or "is that true?" or "r u serious?" or "r u kidding me?"
