"breakdown in communication"?
Does 'breakdown in communication' or 'communication breakdown' mean that 1) there is misunderstanding between two speakers 2) there is no common ground for the two speakers to agree on?

This phrase "communication breakdown" or any equivalent expressions can mean both your suggestions, plus a lot of others. The phrase "what we've got here is (a) failure to communicate" is a quotation from a famous movie, "Cool Hand Luke". In that movie, the warden used that phrase several times.
