It is a set of notes that one worker (usually a supervisor or group leader) gives to his or her replacement at the end of a shift.
It usually contains things that happened during the previous shift that the next person might want to know:
-were there any accidents.
-is the system running correctly.
-is there any unfinished maintenance that needs to be done.
The previous person fills out these notes, sometimes using a pre-printed form and turns them over (gives them) to his/her replacement before the next shift starts.
