First name, given name - if there are no middle names, are the two phrases the same?

Yes, they're all the same. Even if there are middle names, "first name" and "given name" are the same.
One difference:
Usually on official documents, you will see: "Family name / Given name"
Usually in casual conversation, you will hear: "Last name / First name"
