yes, means when somebody looks sick, or does not look good. Can say it when you are sick, or maybe if you are hurt, or when you wake up in the morning, look pale, red eyes, unshaven, not good.

I have also heard it said this way "You look like death warmed over." It means the same. Some one looks very ill.

in general it means when someone looks very tired and unwell.
