I would say
人生で最高だった瞬間と、最悪だった事はなんですか?
jinsei de saikoh datta shunkan to saiaku datta koto wa nandesuka?
but if you wanna know what is going on, you can say
最近どう?元気だった?
saikin dou? Genki datta?
(How have you been? is everything alright?)
