Answering with "I'm okay. How about you?" is perfectly natural. I say this on a daily basis. Other variants would be "I'm fine, thanks. And you?" or "I'm doing well. What about you?" Hope this helps :)

Your question should be, "Can I answer this question like this?" Yes, you can. "How about you?" Is very natural.
