Yo lo diría de esta manera: ¿De qué se conocen? ¿De qué conoces a Carlos?

How do you know each other? = ¿Cómo se conocen? How do you know Carlos? = Cómo conoces a Carlos? But actually the correct form in the last one is: How did you met Carlos? = Cómo conociste a Carlos? hope this helped :)
