Which meaning should I take that as?
1. He's in love with her, so he has a butterflies in his stomach being around her.
2. He's afraid of her because she has extreme mood swings.

It doesn't sound like he is in love with her.
All we know is that he feels uncomfortable in her company. We don't know why, and he may not know why, either. It sounds like he doesn't trust her, or that he suspects that there is something suspicious or unpleasant about her.

Your examples give more detail of the possible reasons for the unease than the original phrase.
The original phrase expresses only that he feels uncomfortable when he is with her. We are not told why. Perhaps the man himself doesn't know why! Sometimes people make you feel uncomfortable and you can't explain it.
