Is 'she is on phone' correct?
Hello guys!
As I am working in a company as an intern, sometimes I have to answer the phone when my colleagues are busy with calling with someone else. Under this kind of circumstances, Can I say ' I am sorry, my colleague is on phone now. Can you call us back later? (What I want to mean here is she is calling with someone else so she is not available at the moment)
and is it also correct to say ' Can you call back later?' or should I add Can you call 'me' or 'us' back?

You'd need to say 'She is on the phone'.
And yes, 'Can you call back later?' is fine.
