A lot more context is needed here. It turns out the line is from the movie "The Bucket List", and yes the character is in hospital for surgery. Seth, you're on the money! :D

I'm guessing here. Could it be that the person is due for surgery which is being carried out by Dr. Eldridge. Dotting up could could be the process of marking the area for surgery, with a pen.
