What does "twist" here mean?

A twist means something unexpected, when something is revealed in a movie or the protagonist is the villain, things like that, that's a plot twist.
In this sentence, the person is saying that they'll answer the questions with an honest twist. Without any context, what I understand is that many people don't answer the questions honestly and he's going to, so that's the twist.
