This is a common phrase for "Are you satisfied/happy now?" or "Do you feel good now?", and is often a sarcasm.
시원하다 can mean "comfortably crisp and cool" (시원한 산들바람), "free and unencumbered"(앓던 이 뺀 것처럼 시원하다), or "feel satisfied, happy, or vindicated" (속이 시원하다).
So it literally means your inside/guts(속이) feel good/comfortable(시원하다), which is to say you feel as good as can be.
