As Brandon said above, most people will say, "Happy belated birthday!" when they are talking directly to someone and giving a late happy birthday wish. However, you could say "belated happy birthday" under circumstances when you are talking about someone else saying the phrase "happy birthday." Example:
"I prefer people to wish me "happy birthday" on my actual birthday, but I'll always accept a belated 'happy birthday.' "
