Let's follow Route 411 to leave town.
That's the most natural. Since you are leaving town, you must be in town and everyone knows what town you are in, so you don't need to specify it by adding "the".

I agree with Mohave, the article ‘the’ is not needed in this sentence. Omit ‘the’ and the sentence will be natural.
